Raise Youth Mentoring - Consent Form

Dear Parent / Carer,


Raise would like to invite your child to participate in our Raise Group Mentoring program which will run at your child's school this year.


The program matches trained volunteer mentors with students for mentoring for the duration of terms 2 and 3 where they focus on developing individual skills to help cope with life’s challenges. To find out more please visit https://raise.org.au/youth-mentoring/in-school/parents-carers/. The program is delivered face-to-face at school. In exceptional circumstances where students are unable to attend school, Raise may temporarily deliver sessions online using our secure, custom-built platform. All delivery will occur in partnership with the school and will follow the Raise Code of Conduct and Youth Safety Framework, both available on our website: https://www.raise.org.au/about/commitment-to-safety


You may already have received some information about this opportunity from your school.


Participation in this program includes taking part in evaluation activities such as surveys (online) and possibly a focus group at school. These will occur during the program and occasionally up to 24 months afterwards and may take place during class time. The results help us improve the program and understand its ongoing impact. You can find examples of questions here


Raise may use the details provided in the surveys or focus groups in communications about program experience. Where this occurs, any identifying details will always be removed to protect the privacy of students and ensure an individual student's experiences cannot be recognised. Or attributed to them.

This address will be used if materials need to be sent to the student, especially in the event of COVID 19 restrictions preventing school attendance

Photography and Image Use 


Raise sometimes captures non‑identifiable images during program sessions - for example, photos showing the backs of students’ heads, hands, or wide shots where individuals cannot be recognised. These images may be used in Raise communications such as reports, marketing materials, presentations, or partner updates.


Raise will never take or use identifiable images of your child without obtaining specific, written consent through a separate form.
